If money issues are stopping you from getting home after a stay in hospital or a care home, we may able to help you.
Our Individual Discharge Fund can provide money to help you return home. Examples of some of the things we can help with are:
- money for a taxi
- paying a bus, taxi or train fare for a relative or friend to come and look after you for a weekend
- topping up empty gas and electric meters
- getting fresh food
Ask for support
If you think money issues are stopping you from leaving a hospital or care home, please speak to one of the ward or home staff. They will be able to refer you to the scheme, which can give you money to help settle you back home.
