About
We have six self-contained properties across the area, all furnished to a high standard. Stays are fully funded for up to 20 weeks, depending on individual circumstances, and come with a person-centred care and support package tailored to each resident.
Our Step-Up Step-Down Practitioner offers both practical and emotional support, helping residents regain independence and prepare for a smooth transition home.
How to access the service
Referrals can be made by hospital clinicians or other health professionals. To ensure suitability for the service, please provide a clear exit plan for the customer at the point of referral.
