Age UK Bath & North East Somerset: Home Independence Support

Home Independence Support (HIS) provides short-term support for people who need help at home following a hospital stay or to prevent avoidable hospital admission during a period of illness or recovery. The service offers practical and emotional support to help people feel safe, confident and independent at home, and to connect with local support where needed. Referrals are available via the Community Wellbeing Hub and existing professional and self-referral routes.

Home Independence Support (HIS) offers short-term, practical and emotional support for people who need a bit of extra help at home following a hospital stay, or during a period of illness or recovery when additional support can help prevent avoidable admission.

We support people to feel safe, settled and confident at home. This may include help with everyday practicalities, checking that basic needs are met, and offering reassurance during a time that can feel unsettling or overwhelming.

Alongside practical support, we know how valuable it can be to have a friendly, trusted person to talk to. Our team can provide a listening ear, help you think through next steps, and connect you with information, advice and local services that may be helpful as you recover.

Support is tailored to individual circumstances and provided for a short period, with the aim of helping people regain confidence, maintain independence, and reduce the need for further hospital stays.
