Connections Day Service
Our service focuses on providing opportunities for adults with a Learning Disability, who require a high level of support to learn new skills, gain confidence and learn independent living skills.
We support adults over 18 years of age.
Buildings Based Day Services support adults with learning disabilities to develop skills and access opportunities for health and wellbeing, communication and daily living skills. Our service focuses on providing opportunities for adults with a Learning Disability, who require a high level of support to learn new skills, gain confidence and learn independent living skills. The service provides reliable, practical support that enables service users to have increasing control over their daily lives, achieve and maintain maximum possible independence and prevent social isolation.
Service users are supported to achieve their full potential and plan to achieve their chosen lifestyle, leading purposeful lives within their community. Activities have a clear purpose, are related to the needs and choices of the service user and are responsive to changing needs.
Alongside our building based services we have a number of community based activities.
When an individual’s needs define that they need a different type of service which will progress them into work, education or training, we liaise with colleges in the Vocational Hub and named Social Workers, to support individuals to achieve their goals.
