Bath & North East Somerset Wellness Service – Everyone Health

Enables people to live well by supporting them to address factors that influence their emotional and physical health and wellbeing. The service aims to work with communities and individuals to increase motivation, skills, capacity and resilience to improve health.

Stop Smoking Service 

12-week Smoking Cessation programme including 1-2-1 or group behavioural support and Nicotine Replacement Therapy for anyone who smokes or uses any other form of tobacco aged 12 years and above. 

For people who don’t want to quit or are not ready to quit, the Swap to Stop scheme, where people will be offered to switch their cigarettes for a FREE vape exchange.

Wellbeing Coach Service 

Supporting people to create a personal health plan to achieve their desired health goal to improve their health and wellbeing. This is for clients who have a specific health related goal they'd like to achieve, for example needing support with weight loss, increase in physical activity, reduction in alcohol consumption, or improving access to local community groups and activities. It will support them to engage with health improvement opportunities, and to engage with the local community. 

The programme consists of 6 sessions either in a 1-2-1 or group setting, face-to-face or virtual with a Wellbeing Coach, with follow ups at 6 and 12 months to encourage sustained behaviour change. 

This service is recommended for people over 16 years who require support and information to make positive changes to improve their health and wellbeing.

Emotional Wellbeing Coach Service 

An Emotional Wellbeing Coach service to support people experiencing poor emotional wellbeing to create a personal health plan to improve their emotional health and wellbeing. This is for clients who have a specific health related goal they'd like to achieve, for example needing support with weight loss, increase in physical activity, reduction in alcohol consumption, or improving access to local community groups and activities. It will support them to live independently in their own homes, engage with health improvement opportunities, and to engage with the local community. 

The programme consists of 6 sessions either in a 1-2-1 or group setting, face to face or virtual with an Emotional Wellbeing Coach, with follow ups at 6 and 12 months to encourage sustained behaviour change. 

This service is recommended for people over 18 years who are experiencing poor emotional and mental wellbeing. Please note this service is not suitable for those diagnosed with serious mental illness, a primary diagnosis of a learning disability or those needing home or personal care services. Our Coaches are not specialist mental health practitioners, if you are in need of mental health support, please contact your GP or local mental health service.

Physical Activity Service 

A Physical Activity service to support people currently undertaking low levels of physical activity, to create a personal health plan to increase their physical activity levels. We will be working with local partners to offer a wide range of 1-2-1 or group based physical activity opportunities over a 12-week programme. Follow ups will be completed at 6 and 12 months to encourage sustained behaviour change and maintenance of physical activity levels. 

This service is recommended for people over 16 years who are currently inactive doing less than 30 minutes of moderate intensity physical activity per week. The service will be tailored for expectant mothers, older people, those who are overweight or living with obesity or living with a managed long-term condition or disability. 

Exercise Referral Service 

Everyone Health will be working in collaboration with Greenwich Leisure Ltd (GLL) and Dragonfly Leisure Trust (DLT) to offer all referrals for Exercise Referral services a 1-2-1 behaviour change consultation prior to starting a programme. This service will require a Health Care Professional referral before we can make the onward referral to the leisure centres. 

This service is recommended for people over 16 years with long-term health conditions to move more to improve their health and wellbeing.

NHS Health Checks & CVD Checks Service 

General Practices will continue to deliver NHS Health Checks directly under direct contract with B&NES Council. In addition, from the 1st April 2025, Everyone Health will be responsible for delivering outreach 

Outreach NHS Health Checks and CVD checks for those less likely to take up the offer of a check through their GP surgery.
