<p>We will work with you to identify and address your physical, practical and emotional needs through a personalised plan, and support you to rebuild your life after stroke.</p><p>We offer:&nbsp;</p><ul><li>Home visits and/or regular telephone calls;</li><li>Communication support;&nbsp;</li><li>1:1 or group based Leaflets, factsheets and information;</li><li>Information on Stroke risk and secondary prevention;</li><li>Assistance with accessing local leisure, social activities;&nbsp;</li><li>Carer Support Benefit Advice Signposting to other organisations that could help;</li><li>Aphasia Choir Peer Support Group</li></ul>
