Before you start

For child or young person-specific requests, you must have agreement from the person with parental responsibility to engage with the SEND and AP Advice Service. Please download and complete the Agreement to engage form, and submit this as part of the online form. If you do not upload a completed Agreement to engage form, there will be a delay in processing your request. 

Provide brief details including the reason for your request and the advice service will get back to you. 

Warning There is no save and return function on the Contact the SEND and AP Advice Service online form. Once started, the whole form should be completed, or it will be lost. Please ensure you have completed an Agreement to Engage form ready to upload.
